H Dani,
Since the title says "left-hand chords" the music should be a single line
beginning with the measure number 1 and the left hand sign or perhaps just
the music (no hand sign or clef sign) and a number 1 sign indicating the
first measure of the exercise. 
All the chords would read from the bottom up, so the first chord would be a
third octave, quarter note E with interval signs for a third and sixth. The
second chord is a third octave F with intervals of a third and fifth, etc.
The ink-print music shows the higher notes of the chords printed in the
treble clef, or right hand of a piano score, but again, these are exercises
for the left hand so all the notes should be shown as complete triads and
not broken up.

The chord names or symbols would be in the line beneath the music, with the
cap sign of each chord name (in literary code) aligned with the quarter note
beginning each chord.

Hi, Everyone.
I'm taking a basic keyboarding class from Berklee, and we're learning 
about chords being played in the left hand. I've attached the .pdf of 
the week's exercises and need a sighted person to have a quick look at 
the first one and tell me how it should look in braille re measure 
numbers and hand
  signs.
