[Menvi-discuss] Transcribing a Chords-only Lead Sheet

Hi, Everyone.
I'm taking Berklee's Pop Rock Keyboard class, and one of this exercises 
shows a lead sheet with chords and dynamics, repeats, coda, and D.S. al 
coda.
I've got Richard's Jazz Chord Reading 1 book that gives me an idea about 
how to handle writing the chords, but don't have any idea how to insert 
the dynamic markings and extra things that show up in a situation like 
this. I've attached a .pdf of the exercise and would appreciate any input.
